Reapplication Process for BLS Practitioners with Expired EMS Certification

To get an expired Pennsylvania BLS certification reinstated...

Either... ...or...

Submit the following to EMSI:

  1. Proof of previous PA certification as a FR or an EMT.

    This may be a copy of a PA DOH certificate and/or copy of wallet card.

  2. Proof of current CPR certification. See list of approved CPR classes that meet the state requirement.

  3. Proof of successful completion of recertification requirements within a 36-month period prior to the date of application.
    • EMT requires 24 total hours with at least 12 CE in M/T category
      • FR requires 16 total hours with at least 8 CE in M/T category
  4. Complete the Criminal History Reporting Form.
  5. Complete the Student Application Form (can be obtained by calling EMSI or visiting any Continuing Education Sponsor). This is the 8 1/2 by 11 inch pink dot form.

ATTN:  Reinstatement Application
Challenge the state written and practical certification examinations.

Call EMSI to get exam dates and to register for exams.

On exam day, you must bring:

  1. Proof of previous PA certification
  2. Proof of current CPR certification
  3. Photo ID
